First Visit Patients

If you are new to our practice then… Welcome! We hope you enjoy your experience and we look forward to seeing you regularly. During your first appointment, we want to make sure that we have everything we need to thoroughly understand your dental needs.

You will be asked to complete a medical history questionnaire that provides us with a great deal of information about you. Your current dental condition – if there is anything you are concerned about, that you may like to change.
Your previous dental history – any conditions that may affect how we treat you now. A general medical history – often your overall health can affect your dental health and vice versa.

We need to know of any conditions you may be suffering from and any medication you are or have been taking.
This information allows us to monitor your dental health, taking into account any contributing factors and provide you with a solution that will work for your individual needs.
 We will also need to take some x-rays of your teeth and gums to establish a good understanding of any problems that cannot be seen with the naked eye. These are kept on file and referred to when assessing progress or deterioration of your dental health.

 This is also a great time to talk to us about any concerns you may have.

Rest assured that any information and conversations you have with us will remain strictly confidential as will any information you provide us with on our questionnaire. If time allows and the treatment required is not too complex, we will have a discussion to discuss your treatment needs.
